Mobile physical therapy is ideal for those who prefer the convenience of home-based treatment, have difficulty traveling, or want a more personalized 1-on-1 experience.

What should I expect during my first session?

During your first session, we’ll conduct a comprehensive evaluation of your condition, discuss your goals, and create a personalized treatment plan tailored to your needs.

What should I wear during my sessions?

We recommend wearing comfortable, loose-fitting clothing that allows easy movement, such as workout attire.

                      How many sessions will I need?

                      The number of sessions varies depending on your condition and goals. After your initial evaluation, we’ll provide a recommended treatment plan with an estimated number of sessions.

                      Do I need a referral from my doctor to begin physical therapy?

                      Depending on your insurance plan, a referral may be required. We recommend checking with your insurance provider, but we can also assist in determining whether a referral is necessary.

                      Do you offer services beyond physical therapy?

                      Yes, we offer follow-up wellness sessions for patients who have completed their physical therapy and wish to continue improving their mobility and strength.

                      What types of conditions do you treat?

                      We treat a variety of conditions, including fall prevention, total joint replacements, rotator cuff repairs, sports injuries, and more.

                      Insurance Information

                      At Hub Physical Therapy, we strive to make our services accessible while maintaining our commitment to high-quality care. Here's what you need to know about our insurance and payment policies:

                      • We proudly accept Medicare Part B and some Medicare Advantage plans. For patients with these plans, we will bill Medicare directly for covered services.
                      • We are out-of-network with all commercial insurance plans. We offer private pay sessions and provide a detailed superbill containing all neccessary information for you to submit to your insurance for potential reimbursement.
                      Additional Insurance Info

                      In rare cases, patients and their families may seek our services in addition to their Home Health or VNA (Visiting Nurse Association) Physical Therapy Plan of Care. Please note:

                      • Medicare regulations prevent simultaneous use of Part A and Part B services, with Part A taking priority.
                      • Patients choosing to receive additional services from us in these circumstances will incur out-of-pocket costs.
                      • An Advanced Beneficiary Notice of Non-Coverage (ABN) form must be signed by the patient or their guardian/Power of Attorney.
                      • The ABN form informs the patient/guardian that the services may not be covered by Medicare.
                      Good Faith Estimate Info

                      You have the right to receive a Good Faith Estimate for the total expected cost of our services.

                      A Good Faith Estimate (GFE) is a key requirement under the No Surprises Act, ensuring that patients receive a clear and accurate projection of the expected costs for their medical care, particularly for uninsured or self-pay patients.

                      For healthcare providers, delivering a GFE means outlining the anticipated charges for services in a straightforward and detailed manner before care is administered. This includes all associated fees, from the provider's services to any additional costs such as lab work or imaging.

                      The GFE fosters transparency, helping patients make informed financial decisions and avoiding unexpected bills, while also ensuring that providers stay compliant with legal requirements.

                       

                      No Surprises Act

                      The No Surprises Act, implemented in 2022, is designed to protect patients from unexpected medical bills, particularly from out-of-network providers in emergency and non-emergency situations.

                      For healthcare providers, this legislation means they must ensure transparency in billing practices and refrain from charging patients more than in-network cost-sharing amounts in covered situations.

                      Providers are also required to give patients clear and advanced notice about their network status and any potential out-of-network costs. Compliance with the act is essential to avoid penalties and maintain patient trust, as it enhances transparency and safeguards patients from financial surprises.
